Primary Responsibilities

  • Provides clinically competent alcohol and drug treatment services; develops clinical impressions; formulates and reviews treatment plans; plans and conducts individual and group counseling sessions in accordance with identified treatment goals and objectives; participates in drug screen monitoring.
  • Develops and monitors individual treatment plans; provides follow-up contacts with referral sources; conducts collateral contacts with family members/significant others; provides follow-up contacts with clients to promote client engagement in the treatment process; coordinates information which promotes continuity of care when transferring primary client care responsibilities to other agency employees and other providers; identifies additional needed services and makes appropriate referrals.
  • Provides individual case management as needed and/or individual and group counseling under clinical supervision that meets quality requirements set by the Agency and that is within the limits of services permitted by position and scope of practice.
  • Accumulates the required number of hours of clinically allocated time based on Agency standards for position and profession.
  • Consults with multi-disciplinary team, consumer, and referral sources prior to therapeutic discharge and completes all discharge documents, including outcome measures within 30 days of last contact
  • Develops and maintains positive collaboration with other social service agencies serving clients. Provides consultation if needed.

Education, Certifications, and/or Experience

  • Bachelor's Degree or comparable experience in human/social services
  • At least one year of experience providing counseling services
  • At least LCDC II OR LPC or LSW with declared scope in chemical dependency
  • Meets all standards and regulations of the Ohio Chemical Dependency Professionals Board, the Counselor and Social Workers Board of the State of Ohio
  • CPR/First Aid Certifications, De-escalation - TCN will provide upon hire

KEY RESPONSIBILITIES

Duties:

  • Provide group and individual counseling for substance use disorders ranging from mild to severe.Maintain documentation to agency standard (training provided)

MINIMUM QUALIFICATIONS

Education:

  • CDCA or above.
  • Dual licensure a plus

POSITION SUMMARY

This position exists to provide alcohol and drug treatment services to individuals and families who are experiencing chemical dependency and substance abuse problems.

KEY RESPONSIBILITIES

1. Provides clinically competent alcohol and drug treatment services; develops clinical impressions; formulates and reviews treatment plans; plans and conducts individual and group counseling sessions in accordance with identified treatment goals and objectives; participates in drug screen monitoring.

2. Develops and monitors individual treatment plans; provides follow-up contacts with referral sources; conducts collateral contacts with family members/significant others; provides follow-up contacts with clients to promote client engagement in the treatment process; coordinates information which promotes continuity of care when transferring primary client care responsibilities to other agency employees and other providers; identifies additional needed services and makes appropriate referrals.

3. Meets or exceeds standards pertaining to services provided.

A. Accumulates the required number of billable hours of clinically allocated time based on Agency standards for position and profession.

4. Meets or exceeds standards pertaining to assessments and documentation.

A. Provides only those services that have been authorized by Utilization Management and provides services within the authorized frequency and duration of services. Requests additional services when initial authorization has been met.

B. Written assessment of individual's history, mental status, strengths, needs and resources including recommendations for treatment completed within 24 hours of the assessment.

C. Assessment and other documentation will provide sufficient reasoning for diagnosis and the type, frequency, and duration of services provided.

D. Treatment Plan and Discharge Criteria with specific goals and objectives that are measurable are developed with the consumer and signed at time of the initiation of services and at time of each update.

E. Assessment, Individualized Service Plan, Progress Notes and other documentation clearly identifies consumer's input in services by having consumer's and parent's/guardian's (if applicable) signatures when clinically appropriate.

F. Completes progress notes at the end of each session and turn in within 24 hours of completion.

G. Consults with multi-disciplinary team, consumer, and referral sources prior to therapeutic discharge and completes all discharge documents within 30 days of last contact.

5. Develops and maintains quality, accurate clinical charts for all clients served.

6. Develops and maintains positive collaboration with other social service agencies serving clients. Provides consultation if needed.

7. Has no unexcused absences from any assigned clinical and administrative committee meetings and training.

8. Maintains license or registration appropriate to profession and appropriate to job requirements, including appropriate amount of continuing education at all times of employment.

9. Maintains agency-required annual training including, but not limited to, client rights and grievances, safety training, infection control, and others as required by OHMHAS.

10. Remains free from citations by other providers or by consumers for unethical practice and meets standard for own profession.

11. Participates in at least 6 hours of supervision for every 3-month period or meets the requirements of supervision required by license/registration (whichever is greater).

12. Other clinical duties as assigned.

MINIMUM QUALIFICATIONS

1. Education: Bachelors Degree or comparable experience in human/social services.

2. Experience: One year in chemical addictions treatment

3. Certification: CDCA, LPC, LSW, RN with declared scope in CD, CDCA,

or Counselor Trainee (OCSWB); CPR; First Aid; De-escalation.

4. Other: Meets all standards and regulations of the Ohio Chemical Dependency

Professionals Board, the Counselor and Social Workers Board of the

State of Ohio, or the Psychology Board. Demonstrated competencies

for working with the populations served.

KNOWLEDGE, SKILLS & ABILITIES

Knowledge of: Safety rules and procedures, Agency policy and procedures, county area and resources, documentation requirements.

Skills: Computer literacy, interpersonal skills.

Abilities: Lifting up to 25 pounds, use of computer keyboard, sitting for periods of time, ability to see, hear, and communicate information clearly.

WORKING CONDITIONS

Some evening hours, occasional travel generally within county. Able to provide coverage when needed. Strict confidentiality is required. May be exposed to potentially angry/violent clients.
